School Exit Form and Checklist
When students leave secondary education, it is wise to obtain formal confirmation of the date of departure. When completed, this document can then be used by Centrelink and other agencies when assessing eligibility for additional support. In addition, we are providing a checklist of recommended tasks to be completed by the student before they leave school.
